Participation Agreement and Liability Waiver
By registering a player in any program, training session, tournament, or event organized by Alma Fuerte Soccer Club, the parent or legal guardian acknowledges that participation in sports activities involves inherent risks, including physical injury, illness, accidents, permanent disability, or other unforeseen circumstances.
The parent/legal guardian certifies that the participant is physically fit to participate and voluntarily assumes all risks associated with participation in activities organized by Alma Fuerte Soccer Club.
Alma Fuerte Soccer Club, including its directors, coaches, staff members, volunteers, and representatives, shall not be held liable for injuries, damages, losses, or claims arising from participation in club activities, except in cases of gross negligence as provided under the laws of the State of New Jersey.
The parent/legal guardian accepts full financial responsibility for any medical treatment, hospitalization, medication, emergency care, or related expenses incurred by the participant.
Medical Authorization
The parent/legal guardian authorizes Alma Fuerte Soccer Club staff and representatives to obtain emergency medical treatment for the participant in the event of an injury, accident, or medical emergency when immediate contact with the parent/legal guardian is not possible.
The parent/legal guardian assumes full financial responsibility for all medical services provided.
Medical Information and Allergies
The parent/legal guardian agrees to disclose any medical condition, allergy, injury history, treatment, physical limitation, or other relevant information that may affect the participant’s safe involvement in club activities.
Failure to disclose relevant medical information shall remain the sole responsibility of the parent/legal guardian.
Photo and Video Release Authorization
The parent/legal guardian grants Alma Fuerte Soccer Club permission to use photographs, videos, recordings, or images of the participant for promotional, educational, advertising, informational, website, and social media purposes without compensation now or in the future.
Code of Conduct
All participants, parents, and representatives are expected to:
- Respect teammates, coaches, referees, staff members, and other participants.
- Follow all club rules, policies, and guidelines.
- Maintain appropriate behavior on and off the field.
- Represent the club with discipline, integrity, and sportsmanship.
- Refrain from offensive language, physical aggression, threats, or unsportsmanlike conduct.
Failure to comply with these standards may result in temporary suspension or permanent dismissal from the club without refund.
Parent and Spectator Conduct
Parents, family members, and representatives are expected to conduct themselves respectfully during practices, games, tournaments, and club events. Aggressive behavior, threats, harassment, abusive language, or confrontations with coaches, referees, players, or staff members will not be tolerated. Alma Fuerte Soccer Club reserves the right to suspend or remove participants whose parents or representatives engage in inappropriate conduct.
Weather and Schedule Policy
Alma Fuerte Soccer Club reserves the right to cancel, reschedule, modify, or relocate practices, games, tournaments, or events due to weather conditions, safety concerns, field conditions, or circumstances beyond the club’s control.
Personal Property
Alma Fuerte Soccer Club is not responsible for lost, stolen, or damaged personal belongings of participants, parents, or spectators during club-related activities or events.
Refund Policy
Registration fees, monthly payments, administrative fees, and any other payments made to Alma Fuerte Soccer Club are non-refundable unless otherwise stated in writing by the club.
Communication Consent
The parent/legal guardian authorizes Alma Fuerte Soccer Club to communicate regarding practices, schedules, payments, events, announcements, emergencies, and club-related information through email, phone calls, text messages, WhatsApp, or other digital communication platforms.
Privacy Policy
Information collected by Alma Fuerte Soccer Club, including personal information, basic medical information, and payment information, will be used solely for club administration, communication, registration, and operational purposes.
Such information will not be sold or shared with third parties except when legally required or operationally necessary in connection with the participant’s involvement in club activities.
Cookies Policy

Electronic Signature Consent
Submission of online forms and electronic registrations constitutes a valid and legally binding electronic signature in accordance with the laws of the State of New Jersey.
Governing Law
This agreement shall be governed by and interpreted in accordance with the laws of the State of New Jersey, United States.
Acceptance of Agreement
By completing registration and submitting this form, the parent/legal guardian confirms that they have read, understood, and agreed to all terms and conditions contained in this agreement.
